MESSA Medical, Dental, & Vision cards

Physical Cards from Dental and Vision

There is not an ID card for Delta Dental of Michigan. You will give your provider: the full SSN of the enrollee, OR the last 4 digits of the SNN, and the enrollee's name & date of birth.

Vision (VSP) only issues cards when a subscriber does not have medical coverage through MESSA. If you are a vision-only member, you will get a card and can use that with your vision providers.

If you have medical coverage with MESSA, you will not receive a vision card. Your provider may locate your policy guidelines and benefits by contacting VSP and providing the full SSN of the enrollee or the last 4 digits of the SSN and the enrollee's name & date of birth.
